Description:
The term chemotherapy which is sometimes shortened to chemo, refers to the use of medications to treat cancer. In chemotherapy, antineoplastic agents are used in an attempt to destroy tumor cells by interfering with cellular functions and reproduction. Chemotherapy is used primarily to treat systemic disease rather than lesions that are localized and amenable to surgery or radiation. Chemotherapy may be combined with surgery or radiation therapy, or both, to reduce tumor size preoperatively, to destroy any remaining tumor cells postoperatively, or to treat some forms of leukemia. The goals of chemotherapy (cure, control, palliation) must be realistic because they will define the medications to be used and the aggressiveness of the treatment plan.

The determination of the necessity for Chemo or hormonal therapy is dependent on the following factors. (Breast Cancer)
· Patient's age and her general condition
· Size of the original tumor
· Involvement of the Lymph glands by the cancer
· Presence or absence of hormone receptors on tumor cells as well as HER2 status
· Genetic and Chromosome abnormalities in the cancer tissue
· Other factors, such as microscopic findings in the cancer tissue
· Extent of the cancer, whether it has spread to other organs.
Following is a list of common rules and guidelines to assess the needs for Chemo or hormone therapy:
Rule # 1: The younger the patient, the more aggressive the treatment, with the goal being to cure the disease. This is achieved by using aggressive chemotherapy.
Rule # 3: All women under age of 70, who are in good state of health, with any stage of breast cancer, regardless of extent of initial disease should be considered for adjuvant hormone therapy or chemotherapy for aggressive disease. Systemic treatment of each patient has to be tailored to her specific situation. '
Rule # 4: All with Her2/Neu positive tumors should be treated with Herceptin, either as part of adjuvant therapy or for treatment of metastatic disease.
Herceptin (trastuzumab)
This drug is one of the most recently released anticancer drugs. Herceptin is a protein, an antibody to a product of a gene that may be present in some patients with breast cancer. Certain Genes play a role in cancers. Oncogene HER-2 may be present in breast cancer cells and produce a protein that would be present at the surface of cancer cells. When this protein is present on the cell surface, Herceptin can detect and bind to it and subsequently cause damage and destruction of such cancer cells. Herceptin is a unique antibody and is similar to Rituximab that is used in treatment of lymphomas.
This drug is approved for treatment of metastatic breast cancers that do express the HER-2 gene induced molecule on their surface. Clinical trials with this drug have shown objective responses in 20-25% of patients. Treatment can result in durable responses and in some patients lasting beyond two years.
Herceptin (trastuzumab) is usually given at your doctor's office or clinic. You take Herceptin through an intravenous (IV) infusion, which means that the drug comes through a needle that your nurse inserts into a vein. Herceptin can also be given through a central line or a port, which are two different devices placed under the skin to make it easier to receive medications that are given through an infusion. Talk to your doctor about which option is best for you.
Infusions will usually last between 30 and 90 minutes, depending on the dosage you are receiving, how frequently you are getting infusions, and how well you are tolerating the infusions. Infusions may be slowed or stopped if you experience discomfort from the side effects.
The most common adverse reactions in patients receiving Herceptin are fever,nausea, vomiting, infusion reactions, diarrhea, infections, increased cough,headache, fatigue, dyspnea, rash, neutropenia, anemia, and myalgia. Adverse reactions requiring interruption or discontinuation of Herceptin treatment include CHF, significant decline in left ventricular cardiac function, severe infusion reactions, and pulmonary toxicity.
Procedure:
1. All chemotherapeutic drug should be prepared according to package insert in class II BSC. Aseptic technique should be followed.
2. Personal protective equipment includes disposable surgical gloves, long sleeves gown and elastic or knit cuffs. Protective eye goggles if no BSC to minimize exposure.
3. Wash hands before and after drug handling.

Drug Administration
Intravenous – It is the most common method of administration of cancer chemotherapy. May be given through central venous catheters or peripheral access. Absorption is more reliable. This route is required for administration of vesicants and it also reduces the need of repeated injection. Because the I/V provides direct access to the circulatory system, the potential for infection and life threatening sepsis is a serious complication of I/V chemotherapy.

· Teach the patient on how the procedure is done.
· A number of medical tests are done before chemotherapy is started. The oncologist will determine how much the cancer has spread from the results of x rays and other imaging tests and from samples of the tumor taken during surgery.

· When a chemotherapy treatment takes a long time, Advise the patient that he may prepare for it by wearing comfortable clothes. Bringing a book to read or a tape to listen to may help pass the time and ease the stress of receiving chemotherapy. Some patients bring a friend or family member to provide company and support during treatment.
· Advise Sometimes, patients taking chemotherapy drugs known to cause nausea are given medications called antiemetics before chemotherapy is administered. Anti-emetic drugs help to lessen feelings of nausea. Two anti-nausea medications that may be used are Kytril and Zofran.
Other ways to prepare for chemotherapy and help lessen nausea are:
· Eat and drink normally until about two hours before chemotherapy.
During the Procedure
· Prepare measures when administering chemotherapy.
· Use gloves when handling chemotherapy drugs and excretions from clients receiving chemotherapy.
· Wear disposable long-sleeves gowns when preparing and administering chemotherapy.
· Use Luer-lock fittings on IV tubing used in delivering chemotherapy.
· Observe the 10 Rights
After the Procedure
· Dispose of all equipment used in chemotherapy preparation and administration in designated containers.
· Dispose all chemotherapy wastes as hazardous materials.
· Monitor client for symptoms of anaphylactic reaction Urticaria (hives), pruritus (itching), sensation of lump in the throat, shortness of breath.
· Asses for electrolyte imbalances.
· Teach client and family to report excessive fluid loss or gain, change in level of consciousness, increased weakness or ataxia, paresthesia, seizures, persistent, headache, muscle cramps or twitching, nausea and vomiting/diarrhea.
· Prevent extravasation of vesicant drugs, implement measures to treat extravasation of vesicant medications if it occurs.
· Teach the client to increase fluid intake to 2500 to 3000 ml/day unless contraindicated.
· Assess for signs of bone marrow depression: decreased White blood Cell and Red Blood Cell, granulocyte, platelet counts.
· Assess for signs of bleeding and infection.
· Monitor for signs of renal insufficiency.
